Patients, carers and healthcare professionals can report a suspected adverse event to AFYX Pharmacovigilance.
To help the safety team assess a report, AFYX’s original reporting form requested details about the reporter and whether they are a healthcare professional; who experienced the event; country and age; product, batch and indication; treatment start date, dose, frequency and whether use continues; the suspected event and its start date; any other relevant illness; and whether AFYX may contact the reporter.
